Support Options can assist you with recovery at home after a hospital stay through the Hospital to Home program.

Hospital to Home

Following discharge from a private hospital, the Hospital to Home program at Support Options can assist with arranging short term support services for people:

  • Aged 65+ (50+ Aboriginal or Torres Strait Islander people) who are frail aged or have dementia
  • Aged under 65 who have a diagnosed disability

The support may include short term funding for:

  • Domestic assistance (home care)
  • Personal care
  • Home Maintenance
  • Respite care

Referrals

For persons aged 65 + /50+ Aboriginal or Torres Strait Islander

Referrals are made through My Aged Care by the discharging hospital by phoning ph:1800 200 422 or online at www.myagedcare.gov.au

For persons aged under 65

Referrals can be made directly to Support Options UMFC by the discharging hospital.
