Providing short term support for older people

Support Options can assist carers and families of people who are aged, have a disability or have dementia.

Carers can be family members or friends, they can be partners, parents, children, siblings or neighbours. Some carers provide 24 hour a day help with daily living activities. Other carers look after people who are mostly independent but who need help with some everyday tasks. Carers do not need to reside with the person for whom they are caring.

 

Short Term Support

Support Options provides assistance to people who are:

  • eligible for government funded aged care services
  • live within the Local Government Areas of Alpine, Benalla, Indigo, Mansfield, Towong, Wangaratta and Wodonga
  • require one off or short term support.

Priority is given to those who require assistance with:

  • personal care
  • domestic assistance.

Making Referrals

My Aged Care
Once My Aged Care has been contacted, a referral will be sent to an Assessment Service who will contact you prior to sending a referral to Support Options to organise services. Where personal care is required immediately, My Aged Care can send a referral directly to Support Options for prompt implementation.

For private patients discharging from hospital, the hospital can undertake the referral to My Aged Care.
